Heaven and hell are states of mind. The brain functions for a while after the heart stops, and the "consciousness" is something modern science has only just begun to glimpse, and they are years from understanding (and likely never will unless humankind opens up and learns to except the "spiritual" sciences which have been with us for centuries. Only with the two in tandem can we begin to truly understand our universe and ourselves.) Essentially, I think heaven and hell are the products of the condition of our consciousness.
As for "what happens" well, I've been thinking about that a lot lately. Most recently I considered that maybe the consciousness just goes on creating reality after reality, almost as if nothing even happened, our concepts of the passage of time being reset in order to make it all seem rational. The mind is a tricky thing, and it is very possible. By the same mechanism, one could experience any number of concepts of "afterlife"
If you think about it, this touches on the Eastern teachings on reincarnation too, that the cycle of existences continues until we reach "nirvanah" or the state of enlightenment, which is essentially awareness of the illusion of reality and existence, thus freeing you from the illusion. Oblivion or unity? As much as we like to glorify "oneness" such a state can only exist in the absence of self, meaning you can't be "you" anymore and your awareness is no longer disparate or unique, which is something most people are rather attached to (they'd have to be, otherwise they wouldn't be here!)
One thing I am fairly certain of, there is no one place for all eternity. Such notions are utterly stupid. For one meager lifetime to warrant ETERNAL damnation, or reward, is preposterous. That's like giving a child a life sentence for stealing candy... then bringing him back to life and making him serve the same sentence over and over and over again. I don't care what you did in life, our life spans are ridiculously short in the grand scheme of the universe. No one deserves anything for eternity based on a single lifetime.
Furthermore, eternity is a state of timelessness, so the whole thing is really just a single continuous moment. It is an existence beyond our 4 dimensional concept of reality, so it is difficult, if not impossible for most people to grasp. The point is you can't be any one thing or perceive any one state for the perspective of eternity.
If indeed energy is neither created nor destroyed, and consciousness, spirit, or what have you are energy, then continued transference from state to state, vessel to vessel, is the only logical conclusion.
